Learning from accidents: a call for humanistic ethics (Keynote at EU Rail Safety days)
The sole aim of this investigation is not to blame but to learn. Sounds familiar? And yet, every investigation follows the same trajectory. In this talk, delivered at the European Union Rail Safety Day Conference in October 2025, I make an appeal for humanistic ethics in investigations. Not what, why or how thing went wrong but the who. Unless we understand the person involved in the accident, nothing will change. In this podcast, I take the example of the captain of Costa Concordia who was imprisoned for the capsizing of the ship. There is no learning without declaring our ethics and methodology.
