https://DrBalikian.com Today, we’re taking a deeper dive into the anatomy of the neck. The goal is to define and understand the various structures involved so we can better explain the terminology and techniques used during neck rejuvenation procedures. When we break it down, we think in terms of four distinct layers, each playing a role in function and appearance. The first and outermost layer is the skin. This is the layer most people notice when thinking of aging or skin laxity. Over time, it can become loose or excessive due to gravity, collagen loss, and reduced elasticity. This sagging contributes to visible signs of aging, such as crepey texture or “turkey neck.” During a neck lift, this area is addressed to restore a smoother, more youthful contour. Beneath the skin is the second layer: subcutaneous fat. This soft, cushioning layer acts as a protective barrier. I liken it to a down comforter, it provides padding and insulation over the deeper structures. This fat plays an important aesthetic role too, helping mask outlines and textures of the underlying muscle layer, which could otherwise create a banded or textured appearance. Next is the platysma muscle, a thin, sheet-like muscle that spans from just above the jawline and drapes down to the collarbone. It’s expressive and can move with facial gestures. Over time, it may weaken or separate in the center, forming vertical bands known as platysmal bands. These bands are a key concern in neck rejuvenation. The platysma also has visible striations, like flank steak, which is why the protective fat layer above it is helpful, it hides that texture from the skin’s surface. Beneath the platysma lies the fourth and deepest layer: the deep neck structures. This includes subplatysmal fat (typically not removable with liposuction), the anterior digastric muscles (which can bulge with age), and the submandibular glands, saliva-producing glands beneath the jaw that can sag as we age, contributing to a heavy neck appearance. In a deep plane neck lift, we begin with these deep structures. We reduce the weight on the “hammock” of muscle by trimming bulky glands, contouring anterior digastric muscles, and sometimes minimally reducing midline subplatysmal fat. These changes build a better foundation for neck contour and relieve downward pull on the overlying muscle. Next, we address the platysma with a procedure called platysmaplasty. Here, we tighten, recreate, and sometimes release the muscle to allow vertical elevation. The goal is to reposition it upward and restore the supportive framework that gives the neck a firm, youthful look. We typically preserve the subcutaneous fat unless there’s a clear reason to reduce it. It contributes to a smooth surface and is only trimmed if overly thick and obscuring contour. Finally, we address the skin. Once the deeper structures are corrected, excess skin is gently removed through incisions hidden behind the ears. This allows us to redrape and tighten the skin without over-pulling, creating a seamless, natural result. In summary, neck anatomy is complex but fascinating.
