I will try to describe what we are doing here. For the lower eyelid surgery, we reach to the subcutaneous layer with an incision from just below the eyelashes to the edge of it. When we pass the subdermal layer, there is a muscle that allows us to close our eyes and also membrane that keeps the fats stable which is called as orbital septum. Lower eyelid baggings are formed when the membrane tissue becomes loose. If there is a large accumulation of these fats, the excess fat can be removed or spreading the fats downwards to correct tear trough deformity. 2️⃣►I had told you about the entry points in the lower eyelid surgeries. For patients who do not have loosening of the skin and have eye bagging, we can perform this operation by entering the inside of the eye to receive the herniated fats (i.e.transconjunctival blepharoplasty) and with laser if it is necessary. Since we are working harder with the inside of the eye, we do not prefer to do this with local anesthesia. If a simple procedure such as only skin excision is to be performed, we can make this operations with sedative anesthesia. 3️⃣►If there is only bagging in the lower eyelid without any skin excessiveness, it can be entered with an incision made from the inside of the eyelid and the fat tissue can be removed. The technique that we called as transconjunctival blepharoplasty can be applied. This is a process by entering from the conjunctiva which is a tissue covers the inside of the eye without any scar. Ater the eyelid surgery, the level of the lower eyelids should be well adjusted. With the removal of the excess skin more than need, eyelids may be pulled down and conjunctival shows may appear. Therefore, it is necessary to pay attention to this situation. It is very important that the lower eyelid aesthetics are performed by the experts. Otherwise, serious problems can be arise later. 4️⃣►After lower eyelid surgeries, there is no need to stay in the hospital. Our patients can turn back to their home on the same day. We recommend to take very simple painkillers at home. We give eye drops to prevent swelling and edema. We suggest to apply ice for 10 minutes for every half hour.
