Sporadic lesions on the hands could be multiple. The reasons could be many. It comes and goes. Goes in the favour of hives or urticarial. It also goes in favour of an insect bite, either a mosquito, bed bug or similar things. With itching it could generally be a bite or an infection. In skin is when there is swelling, there is pain or itching or both. Pain is mainly in infection. Itching is in favour of allergies or insect bites.
