Andy Murray, a professional tennis player, underwent hip resurfacing (Birmingham Hip Resurfacing or BHR) surgery in January 2019 to address a chronic hip condition that had been causing him significant pain and impacting his performance on the court. The surgery involved replacing the damaged hip joint surfaces with metal implants. Despite initial doubts about his ability to return to competitive tennis, Murray made a remarkable comeback, gradually regaining his form and competing at the highest level once again. His successful return to the sport has inspired many and highlighted the potential benefits of hip implant surgery in improving quality of life for individuals with debilitating hip conditions. WHAT EXACTLY IS HIP RESURFACING? Hip resurfacing is an alternative procedure to a total hip replacement (THR), that creates new artificial surfaces for the ball and socket. It is an open (not arthroscopic) procedure that involves reshaping the bone of the ball (femoral head) and the socket (acetabulum), to accept 2 new metal implants that will function as the new joint surfaces. Often it is called "Birmingham Hip Resurfacing", after the named implant that was developed in Birmingham, England in the 1990's. The major benefit of hip resurfacing, as compared to THR, is that the bone of the femur is preserved; this makes sense for younger patients, as this bone may be important should further surgery become necessary. Another advantage of hip resurfacing is that the implanted ball size is larger than a THR, providing greater stability. Studies have shown that the activity level achieved by hip resurfacing patients is higher than that of total hip replacement patients, even when matched for age and pre-arthritic activity levels. The implant used for Andy Murray, a hip resurfacing procedure, is not suitable for most people due to several factors. Firstly, it requires a certain level of bone density and quality to support the implant, which not everyone possesses. Additionally, the procedure is typically recommended for younger, active individuals who have specific hip anatomy and are willing to adhere to post-operative rehabilitation protocols. The implant may not be suitable for older individuals or those with certain medical conditions that could affect the success of the surgery. Andy Murray had primary hip osteoarthritis with very good bone stock which made him an ideal candidate for hip resurfacing. Each patient's suitability for the procedure is assessed individually by their healthcare provider based on various factors. WHAT ISSUE DOES METAL ON METAL CAUSE If it’s used with a femoral stem or if the implants are not put in the best position? Metallosis is a condition caused by the buildup and shedding of metal debris, which occurs when metal joint replacement devices rub against each other and release metal ions into the body. Common symptoms of metallosis include hip or groin pain, numbness, and swelling. Metal poisoning can occur when the metal ions enter the bloodstream, leading to more severe symptoms such as cardiomyopathy, cognitive impairment, and nerve damage. Treatment for metallosis may include revision surgery to replace the faulty joint with a different material and removal of the metal debris from the affected area.
