-The patient’s preoperative assessment included a pelvic ultrasound which demonstrated multiple fibroids, including that which appeared to be a 2cm intracavitary fibroid. - Upon introduction of the hysteroscope, a large myoma was indeed noted arising from a stalk on the right side of the uterine cavity. It has been my experience that staying away from this stalk is beneficial as it isolates the fibroid and allows for efficient removal. Excising the stalk causes the myoma to freely spin within the cavity making the case more challenging. -After introducing the resecting device, it was applied to the most distal portion of the myoma. Then, while applying constant lateral pressure into the partially calcified fibroid, a gentle up-down/in-out motion was used to apply the blade to the tissue during resection.
