Facet joints are joints that help connect vertebrae in the spine above and below, allowing the spine to move properly. These joints have cartilage encircled with fluid to allow them to move smoothly. There are also posterior pair facet joints in the back, which make a three-joint structure combining the disc joint and the facet joints. These are the most common joints we see throughout the spine. These three joints are the big movers of the spine. Each segment influences other segments, which is why these can develop what we call facet joint syndrome, where these joints become debilitated and swollen. This can happen in any area of the spine and are named after the area they are located, Cervical Facet Joint Syndrome Thoracic Facet Joint Syndrome Lumbar Facet Joint Syndrome This syndrome is often caused by arthritis-related conditions such as osteoarthritis. This condition is often related to spinal alignment, making the spine go through degenerative phases faster due to being exposed to more weight and pressure in these areas, making them swollen and painful. When speaking of arthritis, the word itself means "painful joint," so osteoarthritis implies a painful joint as a result of deterioration, so it is not like catching a disease; it is the result of degeneration of the spine. Facet joint syndrome is not a systemic disease. It normally affects one area of the spine, often the side that has more weight or pressure on it. It can also affect the nerve endings that go through the spine, which makes its symptoms very widespread. Depending on the severity and degree of nerve involvement, overall health, and age of the patient, facet joint syndrome could be more painful. Also, depending on where the deterioration is located and nerve involvement, such as cervical and lumbar, often cause more symptoms than the thoracic facet joint syndrome. Also, the degree of nerve involvement will determine how much the facet joint issue will start affecting the spine, which can affect anything the affected nerves control, leading to radicular pain and functional issues as these nerves control everything in the body. Other common symptoms are stiffness and reduced range of motion, with difficulty in rotation or twisting movement, changes to gait and difficulty standing straight, and a bend forward position due to the efforts made to alleviate the pressure on the lumbar joints. Correcting the alignment, in this case, is the goal because this will help with posture, flexibility, and pain. This will also prevent further issues or alignment problems from happening going forward. To treat facet joint syndrome, we must understand the underlying cause. Often, it is a misalignment of the spine, the patient's lifestyle, and things they do that increase inflammation need to be addressed as well. Patients can do hot and cold therapy and exercise to help increase strength in the body, but all this would do is help the symptoms, not correct the cause, which is an issue with spinal alignment that, when left untreated, will continue to show and worsen.
