Toric IOL may need re-rotation, though rarely. Tips and tricks are 1) Identify the destination axis accurately. The side port entry is at an ergonomic position to help IOL rotation. 2) Aqueous air exchange. Keep the IOP normal through out the procedure. 3) No visco is needed or used during this procedure. 4) AC depth has to be normal, as both deep or shallow AC increases the resistance for rotation by clasping the IOL between anterior and posterior capsules. 5) Separate the anterior capsular edge from the IOL with a blunt Sinskey hook 6)While rotating, please see that the haptic is not pushed towards the equator, rather pull the haptic away from that segment of equator. This will reduce the resistance for rotation at the equator. 7) Air BSS exchange is done at the end, and ensure that the IOL is settled well, without any kink in the haptic.
