One of the most common misconceptions in eyelid surgery is that all fullness in the upper lid is fat that should be cut out. There is often puffiness in the upper eyelids due to fat bags that need to be removed during an upper eyelid blepharoplasty, or "eyelid lift". However, when there is fullness on the outer part of the upper eyelid, this is often not due to fat, but rather due to drooping down of the tear producing gland (the lacrimal gland). Accidental removal of this gland can produce dry eye. Instead, the gland should be repositioned into the eye socket and restored to its normal anatomic position.
