He clarifies that giving extra chemotherapy after prior neo-adjuvant therapy usually offers no benefit and increases toxicity. For adenocarcinoma, peroperative chemotherapy is preferred, and adjuvant treatment is needed only for T3/T4 or node-positive disease, or R1/R2 resections. He also reviews chemotherapy combinations: paclitaxel + carboplatin for squamous cell carcinoma, and FLOT or CAPEOX for adenocarcinoma, emphasizing patient fitness and tolerance. Dr. Reddy concludes that esophageal cancer management is complex, requiring a team-based approach with surgeons, medical oncologists, and radiation oncologists. Adjuvant therapy is reserved for specific situations, including residual disease where immunotherapy can improve outcomes.
