Hello dear patients. Today we are going to review the case of erosive balanoposthitis during allergy. A young man came to us with the picture that you can see on the photo. I would say that this picture is quite unpleasant, it is with pains, with massive erosions that I can even call ulcers,as they are quite deep. They have areas of white mucous-like deposit, which indicates necrosis of part of the skin, its rejection. It’s no doubt that in such cases everyone wants to lubricate with something and the patient did that. After reading the internet he lubricated it with several ointments – antifungal ointments, anti-allergy ointments etc. As a result, it even got worse, which is natural. First of all, such patients need examination. This particular case was related to intake of alcohol –wine, cognac in large amounts, however, this young man is not an alcoholic. He did not know of any present allergies. In such cases it’s necessary to exclude such things as syphilis, HIV and to check for allergies, to look at culture test from these erosions as they may be repeatedly infected and to check for the main sexually transmitted infections, including herpes, syphilis etc as such rash is quite hard. However, this rash does not look like syphilis because there’s quite a strong tenderness. The lymph nodes reacted a little and as a result of that, no sexually transmitted infections were noted in this patient. In culture test, microflora was normal but in increased concentrations, which is natural. Why it’s natural? It’s because a normal microflora multiplies in the areas with necrosis. Every bacteria likes to eat what is dead, this is a natural feature of bacteria. There was also a very increased level of immunoglobulin E, it was about 2 thousand. This is approximately 35-40 times more than normal. So, regardless of whether the person knew or didn’t know about this situation, he needs a consultation of allergologist in this situation. I would also recommend not only the consultation of allergologist, but to check nutritional status as well as food intolerances because such patients, after refusing these examinations on first stages of treatment, although I think that these examinations should not be done straight away, only after we calm this process down - I agree with this. However, the patients who did not undergo examination by our allergologist, return to me with relapses. They don’t know what to do to avoid these relapses, they have not been checked for food intolerances, they haven’t checked immunoglobulin E for particular products, etc. I’m not going to dig into allergologic part as this is not my profession. So, what is indicated during such manifestations, during such results of analyses? It is necessary to use sodium sulfate, calcium gluconate, rheopolyglucin or some other sorbent. Internal sorbents like activated charcoal are necessary, antihistamine medications are necessary. It’s also necessary to keep a diet, in these cases I recommend to adhere to an anti-allergic diet, particularly if the level of immunoglobulin E is so high. Of course, it’s forbidden to drink alcohol drinks that were the trigger for appearance of such rashes. From external therapy – compresses and wet-to-dry dressings are indicated. The person comes for dropping tubes and injections every day or every second day and I observe this person. This is what treatment consists of. Regardless of such significant clinical manifestations, how much time does initial intensive treatment take? Usually it’s within 2-3 weeks. Regardless of the fact that the clinical picture is quite tough, the condition usually disappears in 2-3 weeks. We almost never use hormones, we use them very rarely. Sometimes we still use them on initial stage but generally speaking, the therapy that we described is enough. The main thing here is not to continue lubricating by ointments, as ointments may increase this process quite seriously. So it’s better to keep hands off there and to trust the doctor who is treating you. It’s no doubt that here we need to look at analyses that I mentioned before as they may have different variations and then the treatment may be totally different.
