EMA and CEA : labels ductal differentiation (present in microcystic adnexal carcinoma and syringomas. Not present in desmoplastic trichoepithelioma or morpheic basal cell carcinoma). 2. Ber-ep4: helpful in differentiating several tumors from sclerosing SCC (present in microcystic adnexal carcinoma, basal cell carcinoma, and trichoepitheliomas. Negative in SCC) 3. CD34 stains stroma of trichoepithelioma - negative in BCC 4. CK20 stains merkel cells, which are a normal bystander in trichoepithelioma, typically absent in BCC 5. BCL2 - diffusely positive in BCC, and along the periphery of trichoepithelioma (your mileage may vary in desmoplastic and morpheic variants) 6. P75 neutrophin receptor - positive in trichoepithelioma, negative on morpheaform BCC Desmoplastic trichoepithelioma: plaque to nodule on head and neck of younger individual. Diagnostic clues: 1. Symmetrical and well circumscribed lesion in mid to upper dermis 2. +/- central dell (depression) 3. Linear and anastomosing cords (1-3 cell layers thick) with sclerotic and collagenous stroma 4. Horn cysts, often calcified with giant cell tissue reaction 5. 2. No retraction, peripheral palisading, and mitotic activity 6. CK20 positive merkel cells, present in DTE, not in BCC 7.
