INTRODUCTION The posterior (psoas compartment) approach to lumbar plexus blockade was first described in 1974 by Alon Winnie.1The term “psoas compartment block” was coined by Chayen in 1976. The psoas compartment block uses a technique in which a needle is introduced into the space between the psoas major and quadratus lumborum muscle and a large volume of local anesthetic is injected to produce anesthesia of most of the lumbar nerve roots and some sacral roots. It produces anesthesia to the lower extremity in the distribution of the femoral nerve, obturator nerve, and lateral cutaneous nerve to the thigh. The psoas compartment block produces excellent postoperative analgesia for hip and knee surgery. When combined with a sciatic nerve block, anesthesia for surgery of the lower limb is possible.2 Because of the introduction of large volumes of local anesthetic in deep tissues, the potential for local anesthetic toxicity is greater, and the proximity of the epidural space to the injection site increases the risk for unintended epidural anesthesia. It may be a difficult block to perform in obese patients and requires a high degree of training and experience to perform properly. The technique is associated with significant patient discomfort due to needle passage through multiple muscle planes; therefore adequate patient sedation is key.
