This episode explores a study from a NHS hospital that compared 'standard' Root Cause Analysis investigations against a new approach called "Learning Teams." Instead of just looking at what went wrong in a single moment, these teams bring in the people who do the work every day to find the flaws in the system, and to focus on improving the things that make for better, safer, and more efficient work.
