Sooner or later we are all going to have to perform an IOL exchange for a patient. This video shows a resident surgeon's first IOL exchange and there are some important lessons to learn. First is that we must carefully dissect the IOL from the capsular bag, then we can remove it with the twist-and-out technique. The capsular bag can then be cleaned and the IA probe can be used to remove the lens epithelial cells which are causing posterior capsule opacification. Finally the new IOL is inserted. If you are doing an IOL exchange for an incorrect IOL power, for example the patient received a +20.0 IOL but ended up with a post-op refraction of +2.00 for best distance vision, so what is the new correct IOL power to implant to achieve a plano outcome?
