What is postpartum endometritis?

Endometritis is a common cause of postpartum febrile morbidity. The infection is polymicrobial, usually involving a mixture of two to three aerobes and anaerobes from the lower genital tract. Most infections are mild and cured with antibiotic therapy. Cesarean birth, particularly when performed after the onset of labor, is the dominant risk factor. The key clinical findings present in most patients are: -Fever -Uterine tenderness -Tachycardia that parallels the rise in temperature -Midline lower abdominal pain The uterus may be slightly soft and subinvoluted, which can lead to excessive uterine bleeding. Additional findings observed in some patients include malodorous purulent lochia, headache, chills, malaise, and/or anorexia. Antibiotics are administered until the patient is clinically improved and afebrile for 24 to 48 hours.
