Cilnidipine (Atelec) – Uses, Side Effects, Mechanism of Action, Dosage, Interactions, Warnings Cilnidipine (Atelec) is a calcium channel blocker used primarily for managing hypertension, particularly in patients with chronic kidney disease or concurrent tachycardia. It offers kidney protection by reducing proteinuria and is distinct in its dual blockade of L-type and N-type calcium channels—leading to vasodilation and reduced sympathetic activity, respectively. This dual mechanism helps lower blood pressure while minimizing reflex tachycardia and peripheral edema. Typical dosage ranges from 5 to 20 mg once daily, often starting at 10 mg. Cilnidipine is generally well tolerated, with side effects including dizziness, headache, flushing, and, less commonly, hypotension and bradycardia. It may interact with antihypertensives, CYP3A4 modulators, and grapefruit juice.
