Case 185: Manual of CTO PCI - How to occlude aneurysmal SVGs

A patient presented with SVG aneurysm and underwent successful PCI of the native RCA CTO, followed by occlusion of the proximal segment of the SVG with an Amplatzer vascular plug. He returned 6 months later with persistent flow into the aneurysmal SVG retrogradely via the distal anastomosis. A covered stent could not be placed as the PDA was arising very close to the distal SVG anastomosis. After placement of 14 coils retrograde flow into the SVG almost completely stopped. In summary, when treating SVG aneurysms, both ends of the aneurysmal SVG need to be occluded to achieve sealing of the aneurysm.
