It plays a critical role in allowing mobility of the sigmoid colon while housing essential vessels and nerves. The root of the sigmoid mesocolon forms an inverted "V"-shaped line that extends from the left iliac fossa to the level of the third sacral vertebra (S3). Understanding its relations is key for surgeries like sigmoidectomy, colostomy, and assessing causes of volvulus or ischemia. Anteriorly, the sigmoid mesocolon is related to the loops of small intestine, particularly the ileum, which may overlie it and obscure its surface during surgical exploration. Posteriorly, it is related to important retroperitoneal structures such as the external iliac vessels, left ureter, left gonadal vessels, and the common iliac artery 🩸. Of great clinical relevance is the ureter, which runs posterior to the apex of the sigmoid mesocolon, making it vulnerable during pelvic surgeries. The superior rectal artery, a continuation of the inferior mesenteric artery (IMA), runs within the sigmoid mesocolon along with sigmoid arteries and accompanying veins. The inferior mesenteric nerve plexus also courses through it, contributing to autonomic innervation of the distal colon and rectum 🔍.
