Sterile corneal infiltrates are small, white accumulations of inflammatory cells in the anterior stroma, typically near the corneal limbus. Though often asymptomatic, they may cause pain, tearing, discomfort, or photophobia when severe. Commonly linked to contact lens use, these infiltrates arise from debris, chemical toxicity, hypoxia, hypersensitivity, or extended lens wear. Staphylococcal exotoxins from eyelid bacteria may also trigger an immune response. Management includes stopping lens wear temporarily and consulting an ophthalmologist to rule out infection. Treatment often involves short-term topical corticosteroids, eyelid hygiene, and control of bacterial flora using antibiotics or oral omega-3 supplements.
