Negative Dysphotopsias can be very frustrating for the patient as well as the surgeon. The patient is obviously having a problem, yet everything looks fantastic on examination. The IOL is well-centered and the patient's vision is 20/20 uncorrected. Yet the patient had complete resolution of her symptoms when I removed this Clareon single piece hydrophobic IOL and replaced it with a 3-piece silicone IOL. The Alcon hydrophobic acrylic IOL is one of the most popular monofocal IOL's implanted worldwide and 99% of patients are happy with their vision. I use it as my standard monofocal IOL. Yet once or twice a year, a patient will report significant dysphotopsias (positive or negative) with this IOL and usually these resolve with IOL exchange. What's your experience?
