In this highly complicated oblique perforating to the cornea injury, a traumatic intrastromal pocket as complicated with significant epithelial ingrowth and visual morbidity. The incisional exploration of the wound, removal of the epithelium via a manual flap-like approach, repositioning and coverage of the wound with a hydrogel cornea sealant (ReSure) is demonstrated. The visual outcome and healing process rewarded this approach fully. Alternative approaches would have included lamellar keratoplasty, extensive PTK (photo therapeutic keratectomy and possibly penetrating keratoplasty. Eventual rehabilitation may include a topography-guided surface ablation, once the epithelial remodeling stabilizes along with the refractive error, and if contact lens use has been excluded.
