When phaco was first started, it was primarily done with a single hand and just the phaco probe in the eye. The idea of using a second smaller incision (paracentesis) to perform two-handed phaco was new and nucleo-fractis was still to be discovered. Fast forward to 1995 when Nagahara showed phaco chop to the world but there were still surgeons who preferred to "bowl it out" which means using the phaco probe to hollow out the nucleus. This takes a lot of phaco power and time and at the end, the nuclear shell tends to collapse on itself. Fortunately, this technique is rare now and we have embraced two-handed techniques of nucleo-fractis. We include this video as we build CataractCoach into a complete ophthalmic video library.
