(EN) Como decía en el video anterior, estos casos por desgracia son cada vez más frecuentes: pacientes jóvenes, no medicados, con alopecias pequeñas que quieren definir su recesión frontal y acuden a clínicas que contratan a médicos mercenarios salidos de los diferentes “masters de trasplante” y que necesitan hacer la “curva de aprendizaje”. Como resultado, líneas antinaturales, baja cobertura y donante maltrecha. El mes pasado por ejemplo hicimos 2 casos similares peores que este. Este paciente en concreto, vino a consulta en 2022. Nos comenta que tenía una recesión leve en frontal por lo que acudió a una clínica de Madrid, en cuya página se puede leer: “Premio Nacional de Medicina 2022 en la categoría Cirugía Capilar, al ser una institución referente a nivel mundial. Es reconocida como una de las mejores clínicas de Europa, Asia y América, por sus tratamientos innovadores” (sic). Le extrajeron 2500 grafts en 2021, de una ventana occipital por lo que el contraste de cobertura en donante es evidente cuando se corta el pelo. En receptora, baja cobertura, inflamación perifolicular evidente (empedrado o cobblestone), dobles en primera línea y dibujo de la línea por debajo del músculo frontal, lo que conocemos como “frente mono”. El paciente, frustrado con dicha línea, acude a un centro de depilación eléctrica para retirar el máximo los injertos trasplantados (efectividad relativa). Se automedica hace 8 meses con dutasteride día por medio y minox oral y comienza a investigar quién puede reparar su trasplante. Se plantea cirugía de reparación con extracción de múltiples en primera línea. Donante: - Temporal: 53 μm ; 82 UF/cm2; 193 pelos/cm2; 2,35 pelos/UF; CV de 10,23. - Parietal: 55 μm ; 75 UF/cm2; 174 pelos/cm2; 2,32 pelos/UF; CV de 9,57. - Occipital: 57 μm ; 93 UF/cm2; 221 pelos/cm2; 2,38 pelos/UF; CV de 12,60. TDA de 239 cm² TDC de 10.148 grafts (a 2,35 pelos/graft). TDHC de 23.848 pelos. Se extraen 942 grafts (801 + 141 frontal ) (2076 pelos + 311 frontal ), a 2,59 pelos/grafts, para 24 cm2 de frontal (32-39 grafts/cm2). Punch: 0.8/0.85 mm. Grafts: I-200; II-171; III-234; IV-152; V-40; VI-4. Frontal: I-28; II-65; III-41; IV-5; V-2. Se presenta resultado a los 5 meses del trasplante con cambio evidente en naturalidad y cobertura. Esperaremos a los 10 meses por si hay que refinar. Se actualizará.
