Hello dear patients. Today we are going to review the case of paraphimosis. A man came to us, he saw edema of foreskin, he moved foreskin behind the glans penis and initially he thought that glans penis will be more dry and less red after reddening. As it's known, so-called vicious circle is formed in this case. What it is? This is when a narrowed foreskin is moved behind the glans penis and this leads to its circular compression, it gets edematous and compresses glans penis and glans penis itself gets edematous and expands after such compression. That’s why it’s called vicious circle - glans penis expands and foreskin narrows as there's quite a large edema as well. As a result, the extreme condition is necrosis of penis, necrosis of glans penis is more common. The reason is there’s a disorder of nutrition. This already relates to emergency conditions. What has to be done here? It’s necessary to put this glans penis back and it’s necessary to do anti-inflammatory therapy. This was done for this patient, at first analyses for sexually transmitted infections were taken as initially, the cause of this edema was reddening and dryness of glans penis. This man only had contacts with his wife. So, analyses were taken from glans penis for bacterial culture test, culture test for fungi and PCR analyses. PCR analyses were also taken from inside the canal. Repositioning was done, sodium thiosulfate, calcium gluconate were prescribed as well as cold compresses, non-steroidal anti-inflammatory medications in the form of suppositories etc. This slightly surprised the patient, I'm always interested in patient's reaction and I always ask them about what we should have done in their opinion, if not repositioning glans penis and prescribing non-steroidal anti-inflammatory therapy. His answer was: "I wanted an ointment to be prescribed to me”. I told him that if we prescribed ointment he would have turned in hospital with cutting of part of necrosed glans penis. If you see the following picture, then about 2 days after such treatment, edema disappeared completely, gardnerella, ureaplasma and candida were found; they initially induced reddening of glans penis and its small edema. Together with this anti-inflammatory therapy, gradually, the treatment of ureaplasma, gardnerella and candida were done, which initially were the cause of such inflammatory reaction.
