This video demonstrates repair of a rhegmatogenous retinal detachment which includes the macula. The macula is the functional center of the retina and provides us with "20/20" vision when healthy and attached. The macula also provides us with the best color vision. All rhegmatogenous retinal detachments are caused by a retinal tear or retinal hole in the retina. In this patient, there are at least 3 tears/holes that lead to the detachment. Other tears are present, but did not cause the retinal detachment. The goal of retinal detachment surgery is to find/identify all holes and tears and treat then with either laser or cryo. This video demonstrates bothy cryotherapy and laser treatment of retinal tears. This is a 25 gauge vitrectomy system. No scleral buckle was utilized. After the 25 gauge vitrectomy is completed and the retinal tears are treated, fluid gas exchange is performed. Essentially, all the fluid is removed from the eye as air replaces the fluid. This allows reattachment of the retina. SF6 gas (my preference) is used to replace the air at the end of the operation. This particular patient will keep his/her head face down for about 24 hours and then remain sitting up for the next several days. Macular vision should improve but may not completely be restored. When possible, we try to repair retinal detachments before the macula becomes detached. Please let me know if you have questions.
