650 - Treatment of Otitis Externa

Ear wax removal is necessary when ear wax build up becomes impacted. Symptoms causes by earwax blockage include: earache (otalgia), conductive hearing loss, blocked ear, vertigo, tinnitus, itchiness and irritation, occlusion etc. The client attended with bilateral Otitis Externa with occluding debris and dead skin. I commenced with the right ear where the occluding debris and dead skin was quite mushy in consistency. This made is difficult to performing microsuction. A jobson horne or ear hook would also just slice through and dissect the occlusion and not necessarily remove. I was reluctant to instil any drops given the client has Otitis Externa but I had no choice but to. This helped to bind the debris and dead skin together, making it possible to performing microsuction. In the left ear, the clients occluding dead skin and debris was extremely dry and crumbly. I removed the lateral occlusion using an ear hook and jobson horne. The impacted and medial debris was removed using microsuction, including a Fine End Gauge to delicately remove some dead keratin off the tympanic membrane. I have referred the client to their GP but in the meanwhile suggested over-the-counter acetic acid for Otitis Externa and not to avoid allowing water to enter his ears.
