This eye appears to have an anterior polar cataract but there is also concern about some calcification of the anterior capsule. The capsule is stained with vision blue and the anterior chamber backfilled with viscoelastic. The rhexis is then started with a needle cystotome. Extending this past the centre of the lens helps create some slack, allowing the flap to be turned over despite a large central calcified plaque. The tear seems to be progressing well before it reaches an area of unrecognised calcification, where it suddenly wants to go out. Refilling with viscoelastic and switching to rhexis forceps helps give more control, and the tear is soon brought back into the right position. The rest of the rhexis is completed with many small grasps of the flap, overcoming several areas where the tear has a tendency to go out.
