This is a case a phacolytic Glaucoma with a free floating Nucleus in a morgagnian Cataract with a loose bag. Nucleus division was challenging in this case as the capsular bag is very thin an loose and cant withstand the stress during nuclear division. So i decided to use the IOL Scaffold technique where in an IOL is impanated into the capsular bag after manipulating the nucleus out of the bag. The IOL behind the Nucleus will act as a scaffold or a protective barrier and minimizes the chances of inadvertent damage to the posterior capsule or the bag during nucleus emulsification. The concept of IOL scaffold technique was originally described by Dr Amar Agarwal for emulsification of the nucleus /Nuclear fragments in the event of a PC tear. The same principle is utilized here in the presence of a loose bag. Adequate amount of dispersive OVD is mandatory for protecting the endothelium.
