The distinction between an external and internal chalazion can be difficult to make. In general, I usually approach most chalazia through an internal approach, but for some that seem to be all external, I will approach it through the skin. The disadvantage of this is that you could get some unfavorable scaring. If I approach it externally, I will make a horizontal incision, with the thought that it will heal better. I do not place any sutures to repair it. A written transcript is as follows: This is Richard Allen at the University of Iowa. This video demonstrates treatment of an external chalazion of the left lower lid. A chalazion clamp is placed over the lesion and the chalazion will be approached externally due to the anterior positioning of the lesion and the thinness of the skin over the lesion. An 11 blade is then used to make a horizontal incision through the skin over the lesion. A chalazion curette is used to gently express any of the internal portion of the chalazion. More adherent material can be excised with Wescott scissors. Since this is a child under anesthesia, additional treatment is performed by injecting a mixture of triamcinolone and dexamethasone in the area of the lesion to decrease the chance of recurrence. At the conclusion of the case, a combination of antibiotic and steroid ointment is placed over the area and the patient will follow up as needed.
