Sri Shankara Cancer Hospital & Research Centre, Bengaluru hosted The International Head & Neck Oncology Conference (COMPLICON) in August 2024. Various guests from all over India and the world were invited to share their expertise and experience on different head and neck cancers. Speaker: Dr. Subramania Iyer Topic: Lessons learnt in Reconstruction of Maxilla and Mandible In this video, Dr. Subramania talks about: a. Evolution of Flap Choice and Recipient Vessel Selection in Maxillomandibular Reconstruction: While fibula remains the workhorse for vascularized bone, understanding alternatives like DCIA and scapula is crucial, especially when primary options are unavailable. b. Fine-tuning Soft Tissue Management in Complex Maxillomandibular Defects: Managing large associated soft tissue deficits often necessitates the use of two separate flaps to avoid compromising vascularity and coverage. Pedicled flaps like DP or supraclavicular flaps are valuable for external coverage, while the principle remains to prioritize adequate tissue volume and avoid single-flap overextension. c. Addressing Trismus and Submucous Fibrosis in Maxillomandibular Reconstruction: Severe trismus is often a relative contraindication to mandibular reconstruction, but in cases linked to submucous fibrosis, releasing contractures (myotomy, coronoidectomy) before or during reconstruction is vital. Postoperative mouth-opening exercises are crucial for achieving functional outcomes in these challenging scenarios. d. Managing Suboptimal and Secondary Maxillomandibular Reconstruction Challenges: Revisiting cases with plate-only or non-vascularized bone reconstructions requires a staged approach, prioritizing contracture release with existing or new soft tissue flaps before considering bony reconstruction. CT angiography for vessel assessment and STL models for planning are essential for successful secondary procedures. e. The Role of Virtual Surgical Planning and Patient-Specific Implants in Maxillary Reconstruction: Virtual surgical planning and patient-specific implants have become integral for precise maxillary reconstruction, enabling inverse planning for optimal dental rehabilitation and addressing structural stability. These technologies improve accuracy, reduce morbidity, and enhance functional outcomes compared to traditional methods. f. Optimizing Condylar Reconstruction and Dental Rehabilitation in Maxillomandibular Surgery: Condylar reconstruction has advanced from passive plates to glenoid arthroplasty and, ideally, patient-specific TMJ replacements combined with fibula grafts for improved function and reduced mandibular drift. Achieving adequate bone height and a suitable sulcus through strategic flap placement or bone augmentation is crucial for successful denture or implant placement.
