Sx of “malnutrition” in RNY/Billroth II (MGB) Described in the late 1800’s Malnutrition after MGB (mini gastric bypass) is not due to short bowel syndrome. The symptoms of malnutrition have been described in RNY (Roux-en-Y) and Billroth II surgeries since the late 1800s, and it can also occur in partial gastrectomies with short BP (biliary pancreatic) limbs. Thomas E. Starzl's 1965 paper, "The blind-loop syndrome after gastric operations," explains that intestinal abnormalities can cause malnutrition and pernicious anemia, and the location and etiology of the intestinal lesion can vary. However, the underlying cause is always stasis and obstructive drainage of the bypassed limb. These cases with stagnant loops and absorption defects are classified as "the blind-loop syndrome." It is important to note that malnutrition after MGB is not due to short bowel syndrome, as some have erroneously claimed. Understanding the underlying mechanisms of malnutrition after various gastric surgeries is critical for developing appropriate treatment strategies.
