Conduction system of the heart - Sinoatrial node, AV Node, Bundle of His, Purkinje fibers Animation The conducting system of the heart is a specialized network of modified myocardial tissue responsible for initiating and distributing electrical impulses that trigger coordinated cardiac contraction — a cornerstone of USMLE Step 1 cardiovascular physiology. This system ensures the heart beats rhythmically and effectively, independent of external neural input. It begins with the sinoatrial (SA) node, located at the junction of the superior vena cava and right atrium, which serves as the natural pacemaker of the heart by spontaneously generating action potentials. These impulses spread through the atria and reach the atrioventricular (AV) node, located in the posteroinferior region of the interatrial septum, adjacent to the tricuspid valve. The AV node delays conduction briefly, allowing for ventricular filling before the signal proceeds. The impulse then travels into the Bundle of His, a specialized tract of conductive tissue that transmits signals through the interventricular septum. The Bundle of His divides into the right and left bundle branches — with the left further splitting into anterior and posterior fascicles. These branches guide impulses to the Purkinje fibers, which distribute the electrical signal rapidly throughout the ventricular myocardium, ensuring synchronized ventricular contraction. Disruption of any component in this system can lead to serious arrhythmias, such as heart blocks or bundle branch blocks.
