This is just basic information to help you understand what’s going on, so you can ask smart questions when you’re talking to a professional about MRI findings. Don’t use it to diagnose yourself or decide on any treatment for your specific case. DISC BULGE VERSUS DISC HERNIATION/PROLAPSE A disc “bulge” is where the outer part of the disc bulges a bit. A “protrusion” or “herniation” is where the inner gel of the disc ruptures through the outer part. WHICH DISC BULGES AND PROTRUSIONS ARE MORE SERIOUS? DIRECTION: If it’s a “paracentral” bulge or herniation, it’s pushing more into the spinal canal which has a bit more space. If it’s a “posterior-lateral” bulge it’s going a bit more to one side and can compress the nerve roots. SEVERITY: Here’s some of the words you might see in a report... ‘PATENT’ = plenty of space for the nerve ‘MILD PROTRUSION’ = still enough space for the nerve to pass by the bulge ‘ABUTTING’ = disc just touching nerve ‘IMPINGING’ = starting to compress the nerve ‘COMPRESSING’ = the nerve is getting crushed, and depending on severity, this can cause permanent damage HERNIATED DISC SYMPTOMS: A nerve that’s irritated by a bulging or herniated disc can cause pain, but when a nerve starts to become more compressed it can progressively cause pins and needles, numbness, muscle weakness, and eventually bladder signs & symptoms if it’s bad enough. WHEN YOU NEED URGENT MEDICAL ATTENTION FOR A DISC PROTRUSION: Always get professional advice for your specific case… but if you have any constant numbness, muscle weakness, or bladder & bowel symptoms go straight to your doctor because the nerves are being severely compressed. If you delay too long, the symptoms could become permanent. If the symptoms are milder and intermittent, the disc bulge may very well just heal by itself over a few or several months. As a disc protrusion heals, it often shrinks slightly, like a scab as it dries & heals, moving it away from the nerve. IS A MILD HERNIATED DISC BAD ? In many cases, a mildly bulging disc can just be an incidental finding, and any back pain, sciatica, or arm pain may be from something else entirely, like referred pain from the spinal joints, or muscles. COMMON MRI REPORT TERMS: ARTHROSIS—Wear & tear of the joints. (Another name for Osteoarthritis.) Very common as we age. HYPERTROPHY—Enlarging of a part of the body (that may reduce space for nerves in the spine). STENOSIS—Reduced space for the nerve to pass (due to things like bone spurs & disc material). DECREASE/LOSS OF ‘DISC SIGNAL’—Dehydration of the disc due to degeneration. (Looks darker on MRI.) EXTRUSION—When disc protrudes and forms a lump of disc material that’s attached to the disc. SEQUESTERED HERNIATION—When some disc material detaches from the disc & can move about. OSTEOPHYTES—Bony spurs that can form, due to degeneration, and reduce nerve space. THECAL/DURAL SAC—A protective sheath, full of fluid, around the spinal cord. SPONDYLOLISTHESIS/RETROLISTHESIS—When one vertebra overhangs another. Common. Can be mild or severe. SCLEROSIS—An area of increased bone density that’s an early sign of degeneration. FACET DEGENERATION—Wear & tear that creates lumpiness on the ‘facets’ (sliding joints of the vertebrae).
