Design and Function of the GraftMax™ Button - CONMED Animation

The GraftMax Adjustable Loop Button combines the clinical ease of use of adjustable loop designs, with the graft fixation strength of a fixed loop button. Its unique dual fixation design incorporates friction and mechanical locking, which has demonstrated a reduction of displacement by 70% compared to other adjustable loop designs.* A titanium button is combined with number five Hi-Fi suture. This ultra high molecular weight polyethylene suture is assembled featuring a locking loop which sits above the locking pocket on the top of the button. The adjustable loop is then created by passing the suture tails through the button’s tortuous path and, finally, under the locking loop. GraftMax buttons have a unique dual lock mechanism that works in unison. The tortuous path of the suture through the button increases friction and reduces force with each turn to just a fraction of the original at the point of locking loop contact. Mechanical fixation is created by the locking loop applying a direct force to the suture tails, compressing and securing the tails into the locking pocket. During initial tension of the graft, the locking loop compresses down into the locking pocket and may travel approximately 2-3mm as it secures the suture tails into the locking pocket. This locking loop travel is confirmation that the GraftMax Button is fully locked. The GraftMax Button is advanced into position by pulling the two solid blue lead suture strands while taking care to keep slack out of the striped adjustable loop sutures. Once the button passes through the bone tunnel, place tension on the graft to deploy the GraftMax Button. Then, pull both striped adjustable loop sutures to advance the graft into the socket. Place tension on the graft to activate the button’s dual-lock mechanism. A visual confirmation of the 2-3mm of locking loop travel may be evident on the arthroscope when initially tensioning the graft. Date on File. TR 14-464, 12/12/14
