A patient underwent stenting of the right coronary artery. Stent was markedly under-expanded despite high-pressure balloon inflations. He subsequently had stent thrombosis, and the stent could still not be dilated. He underwent a third procedure in which high-pressure balloon inflation and the Chocolate balloon failed to expand the stent. However, laser with simultaneous contrast activation enabled stent expansion. The stent under-expansion algorithm includes the following options: 1. High-pressure balloon inflation 2. Plaque modification balloons 3. Laser 4. Atherectomy 5. Subintimal crossing around stent 6. CABG 7. Intravascular lithotripsy (if available) 8. SIS OPN balloon (if available)
