Sealing the clear corneal incision without stromal hydration - The 'Bud-Out' technique

to ensure a watertight clear corneal incision at the end of phaco, most surgeons employ Stromal hydration. This acts by hydrating the stroma surrounding the corneal tunnel like a cuff and approximates the two lips of the corneal valve. The approximated lips then stick to each other just like how the lasik flap sticks on to the stromal bed.When BSS is injected through the side port incision the Anterior chamber gets pressurised and caused the inner lip on the clear corneal incision to snugly close the inner extent of the incision. The closure of the incision can be achieved by manually approximating the two lips of the corneal tunnel with a cotton bud and allowing the dry tunnel to stick together. Further more injection of BSS through the side port will cause the inner lip to seal shut. This can be achieved in most cases without resorting to stromal hydration, with its attendant risk of wound distortion and increasing the risk of developing a DM detachment
