A 16 incision RK operated case presented for cataract surgery. There were possible previous intra operative full thickness incisions and multiple peripheral iridectomies done. These are challenging cases and requires selection of proper incision site, intra operative care to maintain chamber fluctuation and IOP within a reasonable safe limit. Such things have been noted to cause collapse of AC and PC rent in literature. To overcome these problems, operation was done under extremely low parameters. However, during IOL insertion due to wound stretching, one of the RK incision (showing very poor healing of these even years later) opened up. These leaking wounds were sutured. We have taken only the surgical aspect and not the IOL power calculations
