Nonsurgical Facial Slimming - Buccal Fat Reduction? Dr. Zelken offers a surprisingly simple answer

In some ways, the prospect of #nonsurgical facial slimming seems incredibly intuitive and straightforward. Lose weight. Simple as that. Right? Not necessarily, but this may be the case for some. But for many men and women who seek a more #sculpted face, it is not so easy. After all, the topography of the human face is not skin- (or fat-) deep. Facial shape is dictated by bone structure more than anything else, plus subcutaneous fat, not to mention facial musculature, skin thickness, various deeper layers of fat, and other anatomic structures such as the #buccal fat pads. In other words, weight loss only addresses one of these ingredients: subcutaneous fat. Therefore, patients who are at or below ideal body weight may not be able to modulate facial volume through weight loss alone. Alternatively, many patients at or above ideal body weight maintain thinner faces that are relatively spared from volume-related consequences of weight gain. If you feel that weight loss does not seem to impact your face, or you are at or below your ideal weight, read on for guidance*. If you or someone you know desires a #slimmer face, and weight loss is not an option or does not affect you, surgical options exist. Dr. Jonathan Zelken is gaining national recognition for minimally-invasive and relatively #scarless options for facial #slimming and #contour improvement. Some of the more popular options include facial liposculpture with #radiofrequency skin tightening (“#FaceTite”), buccal fat pad removal, #perioral mound reduction, and #buccinator mucosal #myomectomy. More invasive options like #facelift are also becoming increasingly popular amongst men and women of all age groups. Surgical options are typically the most effective, but they are also the most expensive, riskiest, and inaccessible options. Less invasive options, which are nonsurgical in nature, but still invasive, include injection of #deoxycholic acid (“#Kybella”) into the subcutaneous fat. This is a brilliant concept, but 2 or 3 treatments are typically needed to achieve an often-subtle result, the cost approaches that of liposuction, and Kybella can damage skin, nerves, and make subsequent surgery more difficult and riskier. For these reasons, Kybella is offered at the #Zelken Institute for Aesthetic Medicine but is seldom a preferred choice by patients, nurse Miki Omori, or Dr. Zelken. Other options like topical radiofrequency application (“#Inmode #Evolve”) and #cryolipolysis (#coolsculpting) lack adequate scientific merit or carry a sufficiently high-risk profile for rare but problematic complications like paradoxical adipose hyperplasia (the Linda #Evangelista disease). Truly noninvasive options are probably the most common treatment for facial fullness and #chubby cheeks, and may include facial exercises or #yoga, “#mewing”, habitual “duck face”, and weight loss. There is no scientific evidence that behavioral modifications like yoga exercises and facial contortion work, but there really is no risk associated with them, other than looking goofy to passersby. Of course, these strategies are worthwhile for those willing to explore them. Photo editing software and postural contortion, i.e., #filters and sucking in your cheeks may be effective in photos but are inherently inauthentic or transient in nature. Dr. Zelken believes the best noninvasive option, in appropriate patients, is controlled weight loss through diet, exercise, and lifestyle modification. In this video, Dr. Zelken evaluates 3D images of his face before and after 7.4 pounds of weight loss. Although this represents less than 4% of his body weight, he identifies measurable changes in facial fat distribution that deserve discussion. This is certainly not groundbreaking, and probably does not come as much of a surprise, but Dr. Zelken is very impressed by how much 5-10 pounds can influence- at the very least- his face. Specifically, the prominence of the buccal fat pads from a posterior oblique view is visibly less pronounced, and there is measurable fat loss over the superficial #nasolabial fat pad and chin. Although it is unlikely that the buccal fat pad itself has reduced in volume, its prominence may be less noticeable because the overlying subcutaneous fat envelope is thinner. *Note: This does not qualify as medical advice. Weight loss may not be advisable for those who are at or below their ideal body weight. Individual results may vary as everybody loses fat in different ways. Please consult a board-certified plastic surgeon to discuss invasive, minimally invasive, and noninvasive options for facial contour enhancement.
