In a middle-aged Indian female, an incidental ultrasound revealed a single echogenic linear lesion in the liver near the gallbladder but separate from it, with no vascularity on color Doppler. Differential diagnoses include a calcified parasitic lesion, such as schistosomiasis, hydatid disease, or fascioliasis, all endemic to the region. Portal venous gas, fibrotic bands, or portal vein calcification are other possibilities. The lesion’s benign appearance, lack of Doppler flow, and asymptomatic nature suggest a likely chronic or resolved condition, such as a calcified parasite. Evaluation with serological tests, CT, or MRI can confirm the diagnosis and exclude active infection or vascular anomalies. Prognosis is excellent for calcified lesions, typically requiring no intervention. Follow-up imaging is suggested only for symptomatic changes or uncertainty in diagnosis.
