This is a treatment for the most severe form of erectile dysfunction, when pills or even shots won’t work. Let me explain it to you. Here’s a diagram of a penis. [Dr. Spitz holds up a drawing of the penis and its inner anatomy.] So, here’s the outline of the penis that you’re used to seeing… [Dr. Spitz gestures to the part of the drawing that shows the outside of the penis and scrotum.] And within the penis are two cylinders side by side that actually go as far back into the body as they do out. So guess what? Your penis is twice as long as you thought! [Dr. Spitz gestures to the drawing again.] Now, these cylinders fill with blood. The heart pumps blood to them, they get a little bit bigger, and then they get firmer and harder, and that gives you the erection. And in severe cases of erectile dysfunction, they just simply can’t fill up with blood enough to get hard. So what the implant does is it basically sits within these cylinders, replacing them. [Dr. Spitz points to the cylinders, drawn in red on the drawing of the penis and its inner anatomy] Let’s take a look. I actually happen to have a real one right here. Imagine that! [Dr. Spitz holds up a plastic device composed of two soft tubes, a pump and a soft reservoir.] So, these are the replacement cylinders: Two of ‘em, side by side. [Dr. Spitz holds up the soft tubes.] They are inserted within the natural cylinders of the penis through a little incision, either below the penis or above the penis, and then through the same incision is inserted this [Dr. Spitz holds up the small reservoir]: A reservoir that’s filled with sterile water, and this is what is gonna fill up these cylinders. And that reservoir goes behind the abdominal wall. And then, through that same incision, we put in this. [Dr. Spitz holds up the pump.] This is a pump that transfers the water from the reservoir into the cylinders, and it sits down in the scrotum. So, once it’s all been implanted, the man reaches up through the scrotum, squeezes on the pump [Dr. Spitz squeezes on the pump], and it starts transferring the water into these cylinders and whoa! Erection! [The cylinders get firm and pop up into place.] And these cylinders can actually get very firm, and when they’re positioned and pumped … [Dr. Spitz hits the ends of the full cylinders with the palm of his hand.] … really good, firm penetration. So this is how a penile implant works. But one thing: a penile implant is very different from a breast implant in that it does not increase the size of the penis. In fact, it has to fit within the existing size of the penis, and so the end result is about the same or maybe even a just little bit smaller, but with really good function.
