Etiology involves the production of antibodies that block or destroy acetylcholine receptors at the neuromuscular junction, impairing the transmission of nerve impulses to muscles. This autoimmune attack is often associated with thymic abnormalities, such as thymoma or thymic hyperplasia. Clinical features include fluctuating muscle weakness that worsens with activity and improves with rest. Commonly affected muscles include those controlling eye and eyelid movement, facial expression, chewing, swallowing, and speaking, leading to symptoms such as ptosis (drooping eyelids), diplopia (double vision), dysphagia (difficulty swallowing), and dysarthria (slurred speech). Severe cases can affect respiratory muscles, causing myasthenic crises that require emergency intervention. Treatment focuses on improving muscle strength and managing the immune response. Acetylcholinesterase inhibitors (e.g., pyridostigmine) enhance neuromuscular transmission, while immunosuppressive therapies (e.g., corticosteroids, azathioprine) reduce antibody production. Thymectomy, the surgical removal of the thymus gland, can be beneficial, particularly in patients with thymoma. Plasmapheresis and intravenous immunoglobulin (IVIG) are used for rapid improvement in severe cases or myasthenic crises. Complications of MG include respiratory failure during myasthenic crises, increased risk of infections due to immunosuppressive therapy, and potential side effects from long-term medication use.
