Dr. Caroline Stigant and Helen Chiu present their talk, “Introducing BC Renal's Planetary Health Strategy - Caring for People and Planet,” as part of BC Renal and UBC’s province-wide rounds. 0:08:00 – Decarbonization is possible Dr. Stigant begins by noting that large scale change is possible with oversight, providing a “good news” story about how the UK has decarbonized. 0:10:00 – Kidney Care & Sustainability Dr. Stigant notes that sustainability is not just about greenhouse gas emissions, but also biodiversity, equity, values and reciprocity. She goes on to provide an overview of the environmental impacts of renal medications in BC, including a breakdown of which medications are more environmentally friendly than others. She points to a recently accepted paper, “Life Cycle Assessment – A Primer for Kidney Professionals,” which equips care providers with knowledge to integrate environmental sustainability into clinical practice. 0:15:00 – Carbon Emissions & SGLT2 Inhibitors Dr. Stigant highlights a study to assess the carbon emissions associated with the CREDENCE trial of SGLT2 inhibitors. The results show that prescribing SGLT2 inhibitors can lead to a 20% reduction in carbon emissions. 0:20:00 – BCR Planetary Health Strategic Plan 2023-2028 Overview Chiu then provides on overview of BC Renal’s Strategic Action Plan to enable more sustainable kidney care in BC. Foundational principles of the plan include equity, Indigenous wellness, and planetary health. She highlights several projects underway to support this work, including life cycle assessments, reframing projects to account for planetary health, and building planetary health expertise among health care providers in BC, including the establishment of a BC Renal Planetary Health Strategy Working Group. A key goal in the BCR Planetary Health strategy to reduce greenhouse gas emission from kidney care by 40% by 2030. Chiu highlights some ways in which the strategic plans aims to meet this objective (e.g., more virtual meetings, shifting to sustainable procurement, etc.). Examples of projects that support sustainable kidney care include: Kidney Check, Transplant First, PD Assist, Hemodialysis Emergency Response Team, and development of guidelines for facility planning. 0:35:00 – Lessons Learned Chiu provides an overview of key learnings to date (e.g., engagement is key, provincial vs local opportunities). 0:37:00 – Next Steps Chiu provides an overview of next steps for BC Renal’s sustainability efforts, including launching a BC Green Kidney community of practice. 0:39:00 – Ways to Reduce Kidney Care Emissions Dr. Stigant highlights an upcoming study to explore the environmental and socio-economic impacts of kidney replacement therapies. She notes that kidney care is associated with significantly higher carbon emissions compared to other health services. Dr. Stigant emphasizes the high impact of pharmaceuticals and encourages to deprescribe and select medications with a lower environmental impact when possible. She also highlights data from a study showing that reducing inappropriate lab testing can be beneficial, yielding an estimated 3 tonnes reduction in carbon emissions per year. Dr. Stigant highlights feedback from patient partners regarding sustainable kidney care. Lastly, she discusses how more sustainable use of dialysate could be explored. 0:55:00 - Summary and Q&A Dr. Stigant provides an overview of existing and future challenges before taking questions from the audience.
