Reply by Dr. Sandeep, The various options that exist for us are the wedge resection, where a small portion of the lung, that is the tumor bearing portion is removed or lobectomy, where a single lobe of the lung is removed, versus, penumonectomy where the entire lung of that side is removed. Wedge resection is a very simple surgery where Only the tumor area is removed but unfortunately the cancer has a habit of spreading. So there are most often it spreads to the lymph nodes or the rest of the lung area or the lobe or the lymph nodes in the centre of the chest, when w e do a wedge resection, we don’t remove the entire lung at all, we remove only a portion of the lung. So the long term outcomes of the cancer are much worser when we compare with the radical surgery, that is lobectomy. The other extreme surgery is pneumonectomy, where the entire lung of that side is removed. Now is pneumonectomy required in some cases ? Yes it is, because when a large tumor is sitting close to blood vessels or airway, it is required to do a pneumonectomy, but it is required only in selected cases. So whenever it is required to avoid pneumonectomy. It is avoided, because pneumonectomy comes with a lot of problems, as we are removing half or more than half of the lung, the patient tends to develop breathlessness and they have difficulty in coping up with the surgery in the long term. So the optimal choice very often is lobectomy because we are finding a balance when we perform lobectomy. We get the same outcomes as we get the pneumonectomy, or the moreover the full lung as well as avoid the problem related with the wedge resection, were we leaving behind the cancer tissues sometimes.
