Daniel, MD Commentary by: Aaron M. Kosins, MD This international Original Article from Italy by Dr. Valerio Finocchi and co-authors describes an evolution in preservation rhinoplasty: dorsal reshaping and reduction without STE dissection. They suggest that combining multiple endonasal techniques can yield dramatic changes without dorsal tissue dissection. A Commentary by Dr. Aaron Kosins commends the authors on their algorithm for treatment and its 5 essential components: (1) decide whether dorsal preservation is feasible based on the patient’s preoperative front view and dorsal aesthetic lines; (2) decide whether dorsal preservation is feasible based on the patients’ preoperative profile line; (3) diagnose nasal bones as V-shaped, mild to moderate S-shaped, or severe S-shaped by clinical palpation with/without a cone-beam computed tomography scan; (4) perform a 3-level impaction technique in patients with mild-moderate S-shaped nasal bones; and (5) describe and evaluate the use of mosaic osteotomies without dorsal skin undermining to flatten the bony cap (hump) in patients with mild-moderate S-shaped nasal bones. Miser, MD; and Ren  R.W.J. van der Hulst, MD, PhD Commentary by: Caroline Glicksman, MD, MSJ; and Patricia McGuire, MD This International Original Article from the Netherlands by Dr. Miser and co-authors reviewed breast implant explantation and claims of BII focusing on preoperative patient-reported symptoms, the effect of explanation, and the effect of breast reconstruction on these symptoms. A Commentary By Dr. Caroline Glicksman and Dr. Patricia McGuire explore explantation as a potential solution, the need for prospective data and biospecimen data to identify potential serious adverse events related to breast implants, and the need to elucidate potential etiologies. Long-Term Outcomes of Ophthalmic and Retinal Artery Occlusion After Cosmetic Facial Filler Injection Authors: Yun Jeong Lee, MD; and Se Joon Woo, MD, PhD Commentary by: Claudio DeLorenzi, MD, FRCS Commentary by: Ghazala A.D. O’Keefe, MD What are the long-term visual outcomes and complications related to ophthalmic artery occlusion (OAO) and retinal artery occlusion (RAO) after facial filler injections? That’s the focus of this international Original Article from Korea by Dr. Yun Lee and Dr. See Woo. Increased popularity of cosmetic injection procedures in aesthetic surgery make this study relevant to us all and a Commentary by Dr. Claudio DeLorenzi puts it into perspective, reminding us there may be significant variability in the size and location of vessels not only between patients, but also between the left and right sides of the same patient. Ultrasound imaging can be used for emergency treatment of cutaneous emboli since it allows for direct visualization of vessels, but even this is not without variability including changes in arterial diameter in the periorbital vessels in response to temperature, stress, emotion, etc. Another Commentary by Dr. Ghazala O’Keefe adds to the discussion saying a review of case reports in the literature shows a treatment algorithm for acute management of filler-related ocular complications is performed by a diverse group of practitioners, including dermatologists, plastic surgeons, and ophthalmologists, making medical intervention more difficult. She concludes that while retrospective reviews are helpful, to achieve prevention and better education requires prospective evaluation of incidence rates so patients understand the risks and can truly provide informed consent.
