“It’s in this little bubble,” demonstrates Dr. Michael Collins, an ophthalmologist on medical staff of Lee Memorial Health System. It is the smallest FDA approved implant. Called the iStent, it is surgically placed- you guessed it - in the eye. “If you looked out a penny and took out the two numbers on the date of the penny, it’s smaller than those two numbers,” Dr. Collins says. This teeny, tiny tube is used to treat glaucoma, a condition where pressure builds up behind the eye. It can damage the optic nerve and lead to vision loss and blindness if not treated properly. “There’s different reasons for it. It’s usually either you’re producing too much fluid in the eye or more typically fluid is not coming out of the eye quickly enough,” says Dr. Collins. “If the fluid doesn't egress from the eye quickly enough then the pressure builds up.” Most glaucoma patients use eye drops to manage their disease. However managing eye-drops can be a challenge. People forget or don’t get enough medication in the eye. Others complain of redness or irritation. The iStent is a permanent solution that is helping some patients drop the drops for good. “Right now it’s only FDA approved to be done in conjunction with cataract surgery. So what we do, at the end of the cataract surgery, while we’re inside the eye, we put this stent in something called the trabecular meshwork. The stent actually goes into that meshwork and creates a permanent opening.” That permanent opening lowers eye pressure by allowing fluid to pass more easily.
