Unilateral Tonsillar Enlargement: Aetiologies and Evaluation Unilateral tonsillar enlargement or tonsillar asymmetry may not be a simple tonsillitis. It may be due to some aetiologies ranging from simple lymphoid hyperplasia to malignancy. Squamous cell carcinoma is the commonest tonsillar malignancy followed by non-Hodgkin's Lymphoma. Tonsillectomy is commonly indicated in unilateral tonsillar enlargement but it should be undertaken only after proper evaluation.
