Mark D. Stegall, MD, the James C. Masson Professor of Surgery Research, at Mayo Clinic in Rochester, MN, reviews his article appearing in the September 2025 issue of Mayo Clinic Proceedings, where in response to a significantly higher mortality rate in patients with kidney transplant during COVID-19, he and his team examined the reasons behind this phenomenon. Dr. Stegall noted immunization prior to transplantation appeared to be a major advantage to enhance vaccination response and that when immunization must occur after kidney transplant, measuring antibodies against the target antigen might be useful because some patients may develop only low or no antibodies. He recommends if available, heterologous booster may increase immune responses in these patients and concludes the identification of a group of kidney transplant recipients refractory to multiple vaccinations suggests a phenotype (older patients with long pretransplant dialysis) that might benefit not only from novel prophylaxis regimens but also novel approaches to immunosuppression in general.
