Welcome to my facelift webinar, where we discuss the 4D facelift. What is it? Who’s a good candidate? And what will a patient experience throughout their journey? One of the key things to understand is how aging happens. If you observe a young child or a youthful person, you’ll notice the majority of their facial volume is concentrated high up, creating cherubic cheeks, sharp jawline definition, and a clean neckline. The overall face shape resembles a heart: volume at the top and definition at the bottom. As we age, gravity and time cause tissues to sag. Volume shifts downward, leading to fullness along the lower face and cheeks, with laxity in the neck. This is typically when patients say, “I don’t look the way I used to and want to do something about it.” When you come to my practice, your journey starts with meeting my front office to set up an appointment. During the consultation, I begin by reviewing your medical history and surgeries to ensure you’re optimized for surgery and identify any special accommodations needed. Then, I ask a pivotal question: If you had a magic wand, what would you change? Common desires include looking like oneself, appearing refreshed, and aligning outward appearance with inner vitality. Anatomically, patients often request a sharp jawline, a defined neck, and restored mid-face volume. The next step is my physical exam. I assess overall physique, skin type, coloration, solar aging, and tissue drooping, evaluating the mid-face, jawline, and neck. Based on this, I determine the type of lift that suits you. No two 4D facelifts are alike; each procedure is tailored to the individual. After the exam, I share my game plan, outlining the steps and procedures, including the type of anesthesia—either general or local anesthesia with oral sedation. Local anesthesia with oral sedation is my preferred method for most patients. It minimizes bruising and swelling compared to general anesthesia, as it avoids the blood vessel dilation associated with general anesthesia medications. Patients heal quicker and can return to their routines sooner. About 98% of my facial rejuvenation procedures are done this way. The surgical plan typically includes a facelift, neck lift, fat transfer, and stem cell treatment. The facelift targets the mid-face and jawline using techniques like high SMAS or deep plane approaches to ensure natural, tension-free results. For the neck lift, we remove excess tissue, reshape muscles, and address glands to achieve a sharp 90-degree neck contour. Fat transfer replenishes volume lost over time, with fat harvested from the abdomen or thighs, processed, and injected into critical areas such as the tear troughs, temples, and jawline. Stem cells in the harvested fat enhance skin quality by stimulating collagen and elastin production, leading to smoother, healthier skin. Surgery typically takes about three and a half to four hours. Additional procedures, like eyelid lifts or brow lifts, are discussed during the consultation based on your anatomy and aging. Incisions are carefully placed to ensure minimal visibility, allowing patients to wear a ponytail by week three to four without noticeable scars. Post-surgery, I use drains to minimize fluid accumulation and apply a supportive dressing. On day one, the dressing is removed, while drains usually stay in until day three or four. Sutures are removed gradually, with most out by day seven. By day five or six, patients can resume general activities like grocery shopping, and by day eight, they’re ready for social events.
