Removal of COSMETIC IRIS IMPLANT and DMEK

Removal of cosmetic iris implant and DMEK We present a case of bilateral cosmetic iris implant. The patient developed glaucoma in both eyes, corneal decompensation in his right eye and he had decreased endothelial cell count in his left eye. The cosmetic iris implanted was removed from his right eye and DMEK was performed. Postoperative course was uneventful, corneal edema disappeared after surgery and glaucoma is controlled under topical treatment. The patient delayed explantation of the implant from his left eye and developed corneal decompensation as well. He will require to undergo the same procedure in the left eye. The use of cosmetic iris implants (NewColorIris, BrightOcular) to change the eye color has not been approved by FDA nor CE. High rates of severe ocular complications associated with these implants have been reported (corneal decompensation, UGH syndrome, glaucoma, uveitis). Extracción de una prótesis cosmética de iris y DMEK Paciente con implante de prótesis cosmética de iris bilateral. El paciente desarrolló glaucoma en ambos ojos, descompensación corneal en su ojo derecho, mientras que en su ojo izquierdo presentaba bajo recuento endotelial sin descompensación corneal. Extrajimos la prótesis cosmética en su totalidad y luego realizamos la DMEK. En el postoperatorio el edema corneal se había resuelto y el glaucoma estaba controlado con tratamiento tópico. El paciente pospuso la retirada de la prótesis de su ojo izquierdo, que acabó desarrollando también descompensación corneal. Precisará ser intervenido en el futuro con el mismo procedimiento. Las prótesis cosméticas de iris (NewColorIris, BrightOcular) para cambiar el color de los ojos no han sido aprobadas por la FDA ni la CE, por la elevada tasa de complicaciones asociadas como catarata, descompensación corneal o síndrome UGH.
